Custom form fields not displaying

To all,
I am having problems displaying the fields on my custom form on the visit page.
I can access the fields by clicking the forms button, but that is not acceptable.
I am currently using OpenSpecimen v4.3
Any help would be appreciated.

Please attach your form to ‘Visit Custom Fields’ instead of ‘Visit’. This should display the custom fields on the main page. Refer to ‘What are different levels under which forms can be linked?’ section under ‘https://openspecimen.atlassian.net/wiki/x/lwHfBg’ for more details.
